It's not uncommon for games to have unexpected bugs at or shortly after launch. Here are some of the biggest glitches in semi-recent gaming:

Faceless Characters: An bug during the game's release where character faces didn’t load in right, leaving only eyeballs and teeth, creating a funny but mostly scary visual.

Flying Roach: The updated release brought back a legendary classic where Geralt's horse, Roach, would casually stand on top of buildings, walk vertically on mountains, or float completely upside down.

The Tiny Spider-Cube: A highly viral bug where switching suits occasionally glitched Peter Parker or Miles Morales into a small, featureless white cube. You could still swing around NYC and fight crime as a box.
A cool bug that I never heard of that turned into a feature and major staple for the fighting gaming genre:

Developers at Capcom realized that attacking with precise timing allowed players to skip recovery animations and string hits together uninterrupted. They actually chose to leave it in thinking nobody would realize but to their surprise, players started using the “bug” as a technique, turning the skipped sequences into a core part of their strategies.
When Capcom saw this bug had become a crucial part of the players’ gameplay, they didn't patch it out. They ended up officially designing and balancing future games around the mechanic and other fighting games followed their lead.
